How to Vote by Mail

Request an Application Emailed to You

Please fill in your e-mail address below to receive a Vote by Mail Ballot Application. This form must be printed out, filled in, signed and mailed back to the address provided on the application. We DO NOT accept a completed application via e-mail or fax.

If you do not have a printer at home you can request an application to be mailed to your residence or pick one up at our office in Elizabeth (2 Broad Street, Room 113) or Westfield (300 North Ave)


  • Using a black/blue ink pen, mark your selection on the ballot directly to the right of the candidate of your choice.
  • Fill in the oval completely.
  • Do not vote for more candidates than are to be elected. Write-in selections are available in the blank write-in section and the oval indicating a write in must be completely filled in.
  • Pre-printed stickers are not permitted. The voted ballot shall be placed and sealed in the Certificate Envelope, which shall be completed and signed by you, the voter.
  • Then, the entire Certificate Envelope is placed in the pre-paid mailing envelope addressed to the Union County Board of Elections.
  • Be sure to affix your return address before mailing your ballot. Once returned to the Union County Board of Elections, the ballot is optically read and tallied on Election Day.
